The Kostiuk Family Foundation has partnered with The Atlanta Chapter of the ALS.  The Kostiuk Care Grant Progam provides finanacial reimbursement to Georgia patients diagnosed with ALS and PLS to address needs not covered by traditional insurance, Medicaid and Medicare.  The Grant program awards up to $750 per cycle, not to exceed $1,500 annually.
